PRIVACY ACT AUTHORISATION

Pursuant to the Privacy Act 1993 and Credit Reporting Privacy Code 2004

I/we understand that you, Frontier Finance Ltd (or any Frontier Finance nominated financier), are asking me/us for personal information about me/us so as to use Veda Advantage credit reporting service to credit check me. I/we understand that:

• Veda Advantage will give Frontier Finance Ltd information about me/us for that purpose.
• Frontier Finance Ltd will give my/our personal information to Veda Advantage, and that Veda Advantage will hold that information on their systems and use it to provide their credit reporting service.
• When other Veda Advantage customers use the Veda Advantage credit reporting service, Veda Advantage may give that information to those customers.
• If I/we default in my/our obligations to any third party you have successfully arranged my/our finance through, information about that default may be given to Veda Advantage, and Veda Advantage may give information about my/our default to other Veda Advantage customers.
• Frontier Finance Ltd may use Veda Advantage's credit reporting services in the future for purposes related to the provision of credit to me/us. This may include using Veda Advantage monitoring services to receive updates if any of the information held about me/us changes.
• You consent to Veda (or the Ministry of Justice) disclosing to us any information they hold relating to any overdue fines that may be outstanding against you. Do you understand?

I/we confirm we authorise Frontier Finance Ltd to communicate with any Finance Company, Bank, Credit Reporting Agencies, Work and Income NZ, Department of Courts, Police Department, Inland Revenue Department, Accident Compensation Corporation (or any successors of those government agencies listed), my Employers and Landlords (past, present and future), Mortgagers and/or their respective representatives including Solicitors and Real Estate Agencies, Financial and Commercial information as Active Finance Ltd may require for the following purposes:

• Considering and assessing any application for finance, or the assignment of any loan to Frontier Finance Ltd.
• The protection and administration of any security interest in any loan from Frontier Finance Ltd or any assignment of an existing loan to Frontier Finance Ltd.
• To assist Frontier Finance Ltd in the collection and/or enforcement of any loan agreement between me/us to Frontier Finance Ltd.
• To allow Frontier Finance Ltd to have full disclosure of any mortgage or loan balance to which I/we may be party.

To provide any information Frontier Finance Ltd or its representatives may require in response to any enquiry they may make for the purpose set out above. I/we understand this may include furnishing Frontier Finance Ltd or its representatives with otherwise confidential information including my current address and contact phone number(s). I/we further irrevocably authorise Frontier Finance Ltd to furnish any of the individuals or organisations so described, details of my/our application and any subsequent dealings I/we may have with Frontier Finance Ltd as a result of the application and/or third party loan contract.

COMPLAINTS

If you are not satisfied with the service you have received from us you should contact us. We have an internal complaints process and undertake to investigate your concerns promptly and fairly. You may contact us to make a complaint by telephone, by email or in writing.

We are a member of an independent dispute resolution scheme operated by Financial Services Complaints Limited ("FSCL") and approved by the Ministry of Consumer Affairs. We have 40 days to respond to your complaint. If you are not satisfied by our response, you may refer the matter to FSCL by emailing info@fscl.org.nz or calling FSCL on 0800 347257. Full details of how to access the FSCL scheme can be obtained on their website www.fscl.org.nz. There is no cost to you to use the services of FSCL.
